Medication Wellness Program
In an effort to improve health and decrease the risk of hospitalization or the need for a nursing home, we offer our Medication Wellness Programs. These programs are designed for individuals that still live independently and just need help managing their overall health and medications. We have two different options within this program so that you can choose the best one for your individual healthcare needs. Your options for medication wellness include: |

Services Program:
• Reminder calls for each scheduled medication time and easy to use medication carding.
• Bi-monthly or weekly wellness visits and client/family education by a nurse.
• Medication management, including, but not limited to, refills, order changes, communication with physicians/pharmacy/ Home Health (where applicable) and bi-monthly management of medication.
•Head to Toe Assessment on each nurse's visit.
Medication Monitoring Program:
• The Medication Dispensing System allows for multiple reminders, compliance monitoring, and restricted access to medication.
• Bi-monthly or weekly wellness visits and client/family education by a nurse.
• Medication management, including, but not limited to, refills, order changes, communication with physicians/pharmacy/Home Health (where applicable) and bi-monthly management of medication.
•Head to Toe Assessment on each nurse's visit.
